YOUTH CAMP AGREEMENT & LIABILITY RELEASE
This is a legally binding Agreement and Release made by the undersigned individual or the individual’s parent, guardian or custodian, with New Life Missionary Church, herein after referred to as NLMC, for participation in the camp programs and activities.
Rules and Regulations
By participating in the youth camp programs, the individual or the individual’s parent, guardian or custodian expressly acknowledge and agree that:

Camp participant must be 16 years of age or older.

Campers under the age of 18 must have a written consent of legal parent or guardian for the child to participate in the camp.

Campers under the age of 18 must be accommodated by an older brother or cousin.

The individual is responsible for his or her own actions.

The individual is responsible for following NLMC standards of conduct. If the individual violates the standards of conduct the individual will be terminated from the summer camp program with no refund given, and may be denied the opportunity to attend any future NLMC activities.
Standards of Conduct

Harassment, sexual harassment, physical assault, lewd, indecent or disorderly conduct shall not be permitted on the NLMC camp site or any offsite location during camp stay.

Theft or destruction of property shall not be permitted.

Individuals must comply with official orders of all camp authorities.

Individuals are prohibited from the unlawful manufacture, distribution, possession, or use or sale of illicit drugs, drug paraphernalia or alcohol. This prohibition applies while on the camp ground or while participating in any camp activity.
Indemnification and Hold Harmless
The individual or the individual’s parent, guardian or custodian recognizes that at NLMC and its entire staff is not the insurer of the individual’s health or safety, and has no duty to control third parties. The individual or the individual’s parent, guardian or custodian fully accept and understand that there are potential dangers and risks to which the individual may be exposed by visiting the camp site and participating in programs and activities at camp. The individual or the individual’s parent, guardian or custodian therefore agrees to assume all of the potential risks and dangers, whether or not foreseeable, in any way associated with his/her participation in this year's winter camp programs (including without limitation any and all medical expenses incurred resulting from any illness or injury to the participating individual) and associated activities. In consideration of, and in return for the services, facilities, and other assistance provided to the individual by NLMC in this winter camp program and related activities, the individual or the individual’s parent, guardian or custodian, do hereby release and agree to hold harmless NLMC and its staff, its shareholders, officers, directors, employees, and agents, all of which are collectively referred to hereinafter as NLMC, from any and all liabilities, claims, and actions that may arise from injury or harm to the individual, from the individual’s illness, accidents, death, or property loss brought as a result of involvement in programs and activities of NLMC. The individual or the individual’s parent, guardian or custodian understand that this Agreement and Release covers liability, claims, and actions caused entirely or in part by any acts or failure to act of NLMC, including but not limited to the alleged or actual negligence, mistake, or failure to supervise by NLMC. The individual or the individual’s parent, guardian or custodian agree and understand that this Agreement and Release means I am giving up, among other things, the right to sue NLMC for injuries, damages, or losses that the individual or the individual’s parent, guardian or custodian may incur. The individual or the individual’s parent, guardian or custodian also understand that this Agreement and Release binds the individual or the individual’s parent, guardian or custodian, any non-custodial parent, heirs, executors, administrators, and assigns. The individual or the individual’s parent, guardian or custodian acknowledge that they have read this entire Agreement and Release, that they fully understand it, and that they agree to be legally bound by it. They also agree that this represents the entire agreement and that there are no other oral or written promises or representations which in any way modify its terms.
Please note that individual’s work is implicitly the property of both NLMC and the individual jointly. Individuals may be photographed or videotaped during their time at camp and these images are the sole property of NLMC and may be used for publication at any time. Such publication may occur while the individual is at camp, or any period afterwards.
Minor Medical Release
I as the parent, guardian or custodian of the camp participant indicated below, authorize NLMC, its officers, directors, employees, and agents, all of which are collectively referred to hereinafter as NLMC, full authority and permission to take, in instances including, but not limited to, injury, harm or accident, whatever action that is reasonably warranted under the circumstances. I agree that in the event of a medical emergency NLMC may refer the above-named minor to a licensed medical practitioner and/or clinic and hereby consent that such physician, hospital, or clinic may treat the above-named minor in response to the medical emergency. I also hereby authorize that a photocopy of this authorization be accepted with the same authority as this original. I understand that a reasonable attempt will be made to contact the undersigned parent, guardian or custodian before any action is taken. I agree to be financially responsible for all medical attention so authorized or ordered during the individual’s attendance at NLMC.
